Unless exceptional circumstances arise, the practice welcomes patients living within the recognised practice area. The practice will accept patients regardless of their age and sex, their sexual orientation, appearance, ethnic background, social class, political and religious beliefs, and of their health, any disabilities or medical conditions.
New Patients age 16 or over will be invited to attend for a new patient medical. This will be an appointment with a healthcare assistant. When making this appointment you will be given the relevant forms by the receptionist and these MUST be fully completed before you attend and brought with you for your appointment. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date. Please ensure that you attend this appointment as if you do not then you will be asked to register elsewhere,
Named Accountable GP
Under new contractual requirements we are required to inform you of your Named Accountable GP. Unless you have previously been notified otherwise you will find your Named Accountable GP printed on the right hand side of your prescription, or you can ask at reception. PLEASE REMEMBER THAT YOU CAN STILL SEE ANY DOCTOR OF YOUR CHOICE.
